Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).

A renewal inspection was initiated on Monday, February 28, 2022 concluded on Wednesday, March 2, 2022.The director was contacted by telephone and a virtual inspection was conducted. There were 119 children present with 19 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of children records and interview, the center did not ensure to obtain physical examination documentation within 30 days after the first day of attendance.

Evidence: In Child#2's (start date: 6/2/21) record, there was no documentation of a physical examination found. Interview with staff confirmed the physical examination documentation was not available during the inspection.
Plan of Correction: The center provided family with new school entry form to be completed within the month.
