During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 4/22/2025 and completed 4/23/2025/2025. The on-site inspection began on 4/22/2025 at1:20 pm and ended at 3:20 pm. On 4/23/2025 the on site inspection began at 12:15 pm and ended at 2:15 pm. The inspector reviewed compliance in the areas listed above. There were 71 children present and 8 staff on 4/22/2025. The inspector reviewed 7 children?s records and 7 staff records on-site.
This inspection included document review, tour of the facility, interviews, observations and, measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/10/2025. A POC submitted after this date will not appear on the public website.
Violations
6The center requested a Criminal history record search for one state and a search of the sex offender registry from two states for Staff #2, more than two years and six months late. The results had not been obtained at the time of inspection.
The center has not requested a search of the child abuse and neglect registry from one state for Staff #2, who has been employed for more than 2 years and 6 months.
Be sure to add TB results to file once teacher completes her test and update teacher files when new TB test has been taken.
Staff #2 and staff #4 do not have complete documentation to demonstrate qualification for their positions as Lead Teacher.
