During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 4/3/2025 and completed on 4/23/2025 in response to a complaint received by the licensing office on 3/4/2025 relating to allegations of Supervision. The inspector reviewed 2 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today.

Children under 10 years of age always are required to be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.

On 3/4/2025 Staff #1 sent Child #2 to Restroom "A", where Child #1 was already occupying one of the two available toilets.

Staff #1 walked a few feet away from Restroom" A" to briefly assist the children in Restroom "B".

When Staff #1 returned to Restroom "A", Child #2 relayed that an interaction had occurred which Staff #1 was unaware of because Staff #1 had been unable to hear the verbal exchange between Child #1 and Child #2 from her position at Restroom "B".

Staff #2 was at the carpet with the remainder of the children and was also unable to hear the exchange between Child #1 and Child #2 in Restroom A.
Plan of Correction: Staff will continue to supervise children within sight and sound while using the restroom and occupy and supervise one restroom at a time.
