During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/24/2025 and completed on 10/6/2025. The inspector reviewed compliance in the areas listed above. There were 69 children present and 10 staff. The inspector reviewed 6 children?s records and 10 staff records on-site on 9/24/2025. This inspection included document review, tour of the facility, interviews, observations and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
(Repeat Violation) The center must request a search of the Virginia central registry prior to the employee's first day of employment.
Staff #8, who had been employed for more than 10 months, did not have a documentation of central registry search having been requested (cited for this staff at the last inspection).
Staff #3 who had been employed for more than 4 years did not have documentation of results of a central registry search,
Plan of Correction: The results for a search of the central registry was obtained for staff #3 on 9/29/2025 and for staff #8 on 10/1/2025.
Standard 22.1-289.035-B-4
(Repeat violation) The center is required to obtain, prior to employment, background checks from any state in which the individual has resided in the preceding five years. The center did not have documentation of a Search of the Child Abuse and Neglect registry requested from one state or a Criminal record History Report for one (different) state for Staff #7 who is currently working in the center and has been employed for more than 3 years. Both of these background check violations for staff #7 were cited at the last inspection.
Plan of Correction: The out of state central registry results for staff #7 were received on 10/1/2025. The out of stated criminal history search was sent out on September 9, 2025 but results have not come back. Follow up contact will continue with this state agency until the results of the criminal record check are obtained.
Standard 8VAC20-780-160-A
(Repeat violation) Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.
Staff #8 had no documentation of TB screening results. Staff #8 is currently employed and has been working in the center for 10 months.
Staff #9 had no documentation of TB screening results. Staff #9 is currently employed and has been working in the center for over 2 years and 11 months.
Staff #8 and Staff #9's TB Screenings were previously cited at the last inspection.
Plan of Correction: Staff #9 is already scheduled to have a TB screening on 9/26/2025. Staff #8 will obtain a TB screening within 2 weeks by no later than 10/8/2025.
New staff will be required to have a TB screening prior to coming into contact with children in the center.
Standard 8VAC20-780-240-A
Staff must complete the Virginia Department of Education-sponsored preservice orientation course within 90 calendar days of employment, Staff #2 and staff #3 are currently employed and did not have the Virginia Department of Education-sponsored preservice orientation course. The training is more than 3 years and 9 months overdue for staff #2 and more than 2 years overdue for staff #3.
Plan of Correction: Staff #2 obtained the preservice training on 9/28/2025 and staff #3 obtained the preservice training on 9/25/2025.
Standard 8VAC20-780-40-M
The center is required to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions which is dated and kept confidential in each room or area where children are present.
Staff in two separate classrooms identified a child in their class who has a food allergy confirmed that they did not have a posted allergy list in their classroom at the time of inspection.
Plan of Correction: When program administration was asked about the a current written list of all children's allergies, sensitivities, and dietary restrictions to be posted in classrooms, they confirmed that the old lists had been taken down and not yet replaced with the updated list. The updated list was placed in each classroom during the inspection.
Standard 8VAC20-780-560-F
When centers choose to provide meals or snacks they are required to follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
The current week's menu contained 3 separate meal / snack entries that did not contain all of the required food components according to USDA.
Plan of Correction: The program director will review the USDA requirements and cross reference the menu, for the remainder of the week and future weeks, to ensure that each snack and meal contains a full serving of each of the required food components for each snack or meal.
Standard 8VAC20-780-570-E
(Repeat Violation) Prepared infant formula must be refrigerated, dated and labeled with the child's name.
There were 5 undated infant bottles in the refrigerator located in the infant classroom.
Plan of Correction: Parents will be reminded to date infant bottles prior to bringing them in to the center. Staff we be asked to check for dates and add the current date on a piece of tape on each bottle if they find them not dated.
