During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused monitoring inspection was initiated on January 28, 2025 and completed on February 3, 2025. The Department issued a Notice of Intent to Deny Renewal of the License on December 10, 2024, as a result of high-risk violations. The on-site inspection began at 4:35 PM and ended at 5:30 PM. The inspector reviewed compliance in the areas listed above. There were 12 children present and two staff. The inspector reviewed five children?s records and two staff records electronically on February 3, 2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
