Inspector Notes

An unannounced on-site monitoring inspection was initiated on 11/19/2024 and completed on 11/19/2024. Staff records were checked on 12/5/2024. The on-site inspection began at 3:25 pm and ended at 4:30 pm. The inspector reviewed compliance in the areas listed above.
There were 13 children and two staff present staff. The inspector reviewed three children?s records and two staff records on-site at the main office.
This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The file for Child 1, who has attended for approximately two and a half months, has no proof of immunizations.
Plan of Correction: The center will obtain a copy of the immunization record and put it in the child's file.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician within 30 days of the first date of attendance. The file for Child 1, who has attended for approximately two and a half months has no physical examination.
Plan of Correction: The center will obtain a copy of the physical and put it in the child's file.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. The most recent fire inspection available was dated 10/17/2023.
Plan of Correction: The center will get a copy of the annual fire inspection report from the school and keep a copy with their documents.
Standard 8VAC20-780-530-C
There shall be at least two staff members with current certification in first aid on the premises when children are in care. Two staff were on duty during the inspection, and Staff 1 did not have current certification in first aid.
Plan of Correction: The center will schedule Staff 1 for first aid training, and review the schedule to ensure that at least two staff with current first aid certification are on duty when children are in care.
Standard 8VAC20-780-60-A
Each center shall maintain and keep a record for each child enrolled which shall contain the following information: first and last dates of attendance. The files for Children 1, 2, and 3 had no first dates of attendance noted.
Plan of Correction: The center will add the first dates of attendance for these children and all others at the center.
