Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, and a virtual tour of the program.

An initial subsidy health and safety inspection (SHSI) was initiated on October 25, 2021 and concluded on October 26, 2021. There were 22 children present with 5 staff supervising. A code compliance inspection was also conducted on this date.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to obtain documentation of immunization for one child.

Evidence:
1. Three children's records were reviewed.
2. Child B did not have documentation of obtaining immunizations before the first date of attendance. Start date was documented as 09/18/2021.
3. The director confirmed no proof of documentation of immunization was obtained.
Plan of Correction: The parent will be contacted immediately and proof of immunization status will be obtained. Documentation will be sent to the licensing inspector once received.
