Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on July 21, 2025. The on-site inspection began at 11:07 a.m. and ended at 12:13 p.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 31, 2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 updated screening was due over a month ago. The updated screenings have not yet been completed.

After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for: Water supply; Sewage disposal system; and Food service, if applicable. The Report of Environmental Sanitation from the Health Department was a month overdue.

Hazardous substances such as cleaning materials must be in a locked place that prevents access by children. The storage closet that contained bleach, laundry detergent, and paint was unlocked.
