Inspector Notes

An unannounced, on-site monitoring inspection was completed on 09/05/2025. The on-site inspection began at 9:05 a.m. and ended at 11:24 a.m. The inspector reviewed compliance in the areas listed above. There were no children present (teacher workday) and 6 staff were present. The inspector reviewed 5 children?s records and 5 staff records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/22/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. S2?s
screening was completed 12 days late.
Plan of Correction: Cherub will ensure that all future TB tests are within the 2 year timeframe.
Standard 8VAC20-780-240-E
Within 30 days of the first day of employment, staff must complete orientation training in first aid and cardiopulmonary resuscitation. Orientation documentation for S5 did not document this orientation training.
Plan of Correction: Cherub staff will ensure signatures are on documents to show completion.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill. The center did not complete a drill in August 2025.
Plan of Correction: Cherub staff will conduct a drill every calendar month while in session.

Repeat Violation
The center shall maintain written records of children?s injuries that contain all of the
required information. Injury reports dated 03/24/2025, 04/10/2025, and 04/14/2025 did not document the date and time of parental notification and how the parent was notified. Injury report dated 03/24/2025 did not document actions to prevent recurrence.
Plan of Correction: Cherub staff will ensure that all injury reports are filled out to completion.
