Inspector Notes

A subsidy health and safety (SHSI) inspection was initiated on September 20, 2023 and concluded on September 25, 2023. There were 50 children present with 5 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record reviews and staff interviews, the vendor did not ensure staff who work directly with children shall have completed Virginia Preservice Training.
Evidence: The record for Staff #1 (hire date 9-30-2021) did not include documentation of completing the required updated training. The most recent training available for review was dated 10-20-2019.
