Inspector Notes

An unannounced monitoring inspection was started in the center on January 25, 2024 from 3:45 PM 4:40 PM and concluded on January 31, 2024. At the time of entrance, 26 children were in care with 4 staff members. The sample size consisted of 5 children?s records and 4 staff records. Children were observed playing decorating cookies and playing board games.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat
Based on record review and provider interview, the licensee did not ensure the physical examination and dates of immunizations shall include the signature of a physician, his designee, or an official of a local health department

Evidence: The immunization for Child #4 did not include a physician or designated individual?s signature.
Plan of Correction: program manger is working with families to gather all required information
Standard 8VAC20-780-550-P
Based on center documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence that includes all required information.

Evidence: Five out of five injury reports reviewed were missing information to include
staff present during injury, date, time and how the parent was notified of the injury.
Plan of Correction: Senior program manger and area manger are working to have injury reports created that contain all the required information

Repeat
Based on record review and staff interview the center did not maintain a record for each child enrolled which contained all of the required information.

Evidence: Director confirmed the following items were missing from children?s records:
1.The record for child #1 did not include a parents work phone number.
2.The record for child #5 did not include work phone numbers for both parents listed and did not include the appropriate paperwork when a custodial parent requests the center not to release the child to the other parent.
Plan of Correction: Program manger is working with families to obtain the missing required information

Based on record review and staff interviews, the licensee did not ensure staff records contained all required documentation.

Evidence: The record for Staff #2 did not include the emergency contact information and health related information
