Inspector Notes

A 60 day monitoring inspection was conducted in-person on 3/7/2022. The rooms were measured for accuracy from the virtual Initial inspection. 1 staff and 3 child records and injury reports were reviewed. See Violation Notice for missing items.
First aid supplies, emergency supplies, emergency drills and the playground are all in compliance. The inspection report will be typed off site and emailed to the facility for signatures. Based on a review of records, one staff record does not contain documentation of a completed Sex Offender search, or Child Abuse and Neglect search for a residence outside of the state of Virginia in the past 5 years.
Evidence:
*1 of 1 staff records reviewed did not contain documentation of the results of a Sex Offender search for an out of state residence, prior to employment. The result is dated 1/11/22.
* 1 of 1 staff records reviewed did not contain documentation of an out of state Child Abuse and Neglect Search within 45 day of employment. (Staff A - Date of Hire:1/3/22). There is no documentation to show a search was submitted.
Plan of Correction: Moving forward, the results will be on file prior to employment for the Sex Offender search. I will keep a copy of the form mailed off with the date of mailing written on it. I will follow up on the search.

Based on a review of injury reports, the injury report form does not contain all required components.
Evidence:
1 of 1 report reviewed did not contain documentation of the "future action to prevent recurrence"
Plan of Correction: I will make changes to the form to include that component.

Based on a review of records, one child record does not contain documentation of all required components.
Evidence:
1 of 3 records reviewed did not contain documentation of the first date of attendance. (Child 1)
Plan of Correction: I research the 1st date of attendance and complete the form. Moving forward it will be completed on the first day of attendance.
