Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program .

A monitoring inspection was initiated on 1/24/2022 and concluded on 02/07/2022. The director/was contacted by telephone to initiate the inspection. There were 34 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program

Based on record review, the licensee did not ensure a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #1 hire date 08/30/2021 ,there was no results or documentation that a out of state search of the child abuse and neglect registry for the state of California was completed.
Plan of Correction: Form will be obtained by staff member and submitted to California.

Based on record review, the licensee did not ensure a annual fire inspection is completed.

Evidence: The fire inspection provided during the inspection was dated 11/06/2020. The fire inspection is more than a year old.
