Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and on March 3,2022 an in person tour of the program was conducted.

A renewal inspection was initiated on March 3, 2022. The provider was contacted by telephone and an inspection was conducted in person. There were 54 children present, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and licensee interviews, the licensee did not ensure to a obtain a sex offender check prior to the end of the 30th day of employment /service for new agents/board officers. The out of state background checks are required for any person who has lived out side of Virginia within the last five years.

Evidence: Area Manager confirmed, there was not criminal record result available for review for Agent #1 who lived in New Jersey within the last five years. Agent #1, start date 1-1-2022.

Based on record review and licensee interviews, the licensee did not ensure to a obtain a criminial record check prior to employment. The out of state background checks are required for any person who has lived out side of Virginia within the last five years.

Evidence: Staff #1 hire date 10/26/2021. Staff #1 lived out of state (Alaska) in the past 5 years. The center was unable to provide criminal record results . Out state criminial record results should be completed prior to employment.

Based on record review, the licensee did not ensure there was a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
