Inspector Notes
An unannounced monitoring was conducted on April 25, 2023.There were 53 children present with 2 staff members. Additional staff and children arrived during the inspection. Children were observed children arriving to the program in the morning, multiple activities and gym activities.
Two staff records and four children records reviewed . Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and sheltering-in-place drill documentation,
emergency procedures and required posting. Staff records were requested to be reviewed remotley.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Staff#1 hire date was 06/3/2022 and tuberculosis screening was completed 06/06/2022.
Staff #2 hire date was 03/24/2023 and tuberculosis screening was completed 03/29/2023.
Evidence:
The head count sheets and observation maintained by the center for the morning program revealed that the center did not meet the required staff-to-children ratio on the following date:
On April 25, 2023, from 7:30am- 8:30am, two staff were present upon arrival with 38 children and by the time children were dismissed to school their was 53 children with 2 staff .

Child #1 had an allergy and was not listed on the allergy list.

Child #1 had a doctor diagnosed allergy and did not have written care plan from physician.
