Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on September 11, 2024. And the inspection concluded on September 13, 2024. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/25/2024. A POC submitted after this date will not appear on the public website.

Staff who work directly with children shall, in addition to preservice and orientation training required, annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.

Staff #2's annual health and safety update course for 2023 was completed 1 year and 6 months after due date .
Plan of Correction: Senior Program manager will continue to work with staff to get training completed in a timely manner.
