During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9-22-2025 and completed on 9-25-2025. The on-site inspection began at 4:25pm and ended at 5:40pm. The inspector reviewed compliance in the areas listed above.
There were 35 children present and 4 staff.
The inspector reviewed 5 children?s records and 4 staff records electronically from 9-23-25 through 9-25-25. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10-2-2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. Staff #2, has not yet completed this course which is overdue by 4 month.
Plan of Correction: Senior program manger will work with staff to complete trainings in a timely manner.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Child #6 has a medication authorization for 1 medication, and the authorization expired over 2 months ago. The authorizations had not been renewed, and the medication had not been returned or disposed of as required.
Plan of Correction: Program manager will work with family to get an updated medical authorization form.

Each center shall maintain and keep at the center a separate record for each child enrolled that contains all the required information.
1.The record for Child #1, did not have documentation of the complete address for both required emergency contacts to include state and zip code.
2. The record for Child #3, did not have documentation of the employment number for one parent listed and the complete address for both required emergency contacts to include state and zip code.
