Conducted a focused desk review of program director qualifications. An on-site inspection was not conducted. It is the responsibility of the licensee to know and implement current standards and laws. Based on records reviewed, the licensee failed to ensure that 1 of 2 staff records included an acceptable form of documentation of a TB screening. Evidence: Staff #3-The TB test submitted does not include a patient name on the document.

Based on records reviewed, the licensee has failed to ensure that individuals working as a program director meet the minimum qualifications.
