Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
An initial monitoring inspection was initiated on August 19, 2021and concluded on August 19, 2021. The provider was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, programming, special care and emergencies and nutrition. The background checks for 2 applicants and two staff was reviewed along with required Program Director qualifications.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview, the applicant did not have at least one staff who has obtained Daily Health Observation with in the last three years.

Evidence:
During the inspection the applicant #1 confirmed that they do not have documentation of a staff that has obtained .daily health observation of children training.
Plan of Correction: The applicant #1 will review records to locate documentation of daily health observation training. The applicant will have staff take a online class for daily
health observation training if documentation is not located.
Standard 22VAC40-191-40-C-1-A
Based on record review and applicant interviews, the applicant did not ensure to obtain a Search of the Central Registry upon application for licensure.
Evidence: Applicant #1 confirmed, the findings from a Virginia Search of the Central Registry were not available for review during the scheduled initial inspection for Applicant #1 and Applicant #2.
Plan of Correction: The requests have been submitted for applicant #1 and applicant #2 . Applicant #1 have stated phone calls were made to the Office of Background Investigations to get updates on the requests. Verification of the results will be sent to the Licensing Inspector upon receipt.
