Inspector Notes
An unannounced monitoring inspection was conducted on February 8, 2023 from approximately 8:25am -9:30am . Upon arrival to the facility, there were 48 children present 2 staff. Children were playing in multiple station activities , bathroom break and transitioning to leave for school . Four children records were reviewed and three staff records were reviewed. Staff records were reviewed remotley.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview, the licensee did not ensure center obtained documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence: Child #2 record did not contain a immunization documentation. Staff confirmed there was not imnuization document in his record.

Based on observation and staff interview, the licensee did not ensure there was 1staff: 18children ratio for school age elgible up to 9 years.
Evidence:
During the inspection on February 8, 2023, Staff #1 and Staff #2 were present with 48 children at 8:30 am . There were 2 staff to 48 children . The ratio should be 1 staff to every 18 children.
