A monitoring record review inspection was initiated on September 21, 2023 from 4:45pm ? 6:50pm. The inspector reviewed compliance in the areas of administration, staff qualifications and training and background checks. A total of three staff records were reviewed . Information
gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to th program.

Based on reocrd review and staff interview, the licensee did not ensure all staff records obtain all required documents.
Evidence:
Staff #1, Staff #2 and Staff #3 did not have documentation of qualifications for position .
Staff #2 did not have orientation documentation.

Plan of Correction: Area Manager and senior program manager will provide training to quality staff to be lead as well as store documentation in staff files . Orientation has been completed and asses to employee file.
