Inspector Notes

An unannounced monitoring inspection was conducted on October 4, 2023 from 3:30pm-4:40pm. At the time of entrance, 42 children were in care with 2 staff members present. Additional staff arrived during the inspection. The sample size consisted of 5 children?s records and 4 staff records were reviewed during record review prior to inspection. Children were observed during snack time, transitoning in the activities for the day, and bathroom breaks. Repeat violations were found and are identified within this report. Areas of non-compliance are identified on the violation notice.

Based on observation and record review, the licensee did not ensure ratio was maintained for the children, from school age children up to 9 years , 1 staff for every 18 children.

Evidence:

The center did not meet the required staff-to-children ratio on the following date:

October 4, 2023, from 3:40pm to 3:55 pm; there were 42 children with 2 staff.

Based on record review and staff interview, the licensee did not ensure a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
