Inspector Notes

An unannounced renewal inspection was conducted on August 11, 2022 beginning at 7:34 AM and ending at 9:45 AM. There were 54 children, ages 6 months through 10 years old, in the care of 12 staff members. The children were observed playing with age appropriate toys and interacting with staff. Five staff records were reviewed and in compliance. Five staff records were reviewed and in compliance. Two medications were reviewed and in compliance. The first aid kit, flashlight, and battery-operated radio was observed. The emergency evacuation practice drill log was reviewed and in compliance. The most recent health inspection was dated June 28, 2022. The most recent fire inspection was dated June 14, 2022.

Standard 8VAC20-780-160-C
Based on a review of staff files, the center did not obtain documentation of a tuberculosis screening at least every two years from the date of the first initial screening. Evidence: Staff A did not have documentation of a sub-sequential tb screening in their record. The most recent tb screening was dated 3/18/2020.
Plan of Correction: The facility is aware that the staff member's tb screening has expired and has requested that an updated tb screening be conducted. Once completed, the documentation will be placed in the staff's record.
Standard 8VAC20-780-570-E
Based on observation, the center did not ensure that all of the bottles of formula were dated. Evidence: There were two bottles of formula in the refrigerator of the infant classroom that did not have the date on them.
Plan of Correction: The bottles were dated during the inspection. Staff was reminded to check that all bottles have been labeled with the child's name and date.
