Inspector Notes

An unannounced investigation was initiated and completed on 4/9/2025 in response to a complaint received by the licensing office on 4/4/2025 relating to allegations of Supervision of children. The inspector reviewed 1 child record on-site. This investigation included document review, tour of the facility and interviews.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 5/2/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

1. Since 3/19/2025 through 4/11/2025, Child A (4 years old) had over 35 documented behavioral incidents involving other children in the room to include: Spitting on other children, hitting and punching the children, using profanity, exposing their private area to the children, making gun hand gestures toward other children, pulling other children?s hair and calling them names.
2. The center failed to provide the protection needed to the children in the classroom as a result of child A's behaviors.
