Inspector Notes
An investigation was initiated from 11:15 am - 12:00 pm to investigate a complaint regarding supervision and playground areas. Interviews were conducted and documentation was reviewed. The complaint regarding supervision and playground areas is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 3Standard 8VAC20-780-330-A
Based on observation and interviews it was determined that the toddler playground was not designed to protect children from hazards. Evidence: The gate on the toddler playground adjacent to the parking lot has a button mechanism accessible to children that allows access to the center parking lot. On 08/23/2022 at 4:45 pm child A (Age 2) was able to exit the toddler playground by simply pushing a button that allowed the child to open the gate and enter the parking lot.
Plan of Correction: Placed a gate latch that is accessible to teachers and is not as accessible to the children.
Standard 8VAC20-780-340-A
Based on interviews it was determined that staff did not always ensure the care, protection, and guidance of children in care. Evidence: On 08/23/2022 at 4:45 pm there were 10 children in the two year old room being supervised by two staff on the outdoor toddler playground. Staff A was sitting on the climber stairs and Staff B was standing on the sidewalk adjacent to the building. Child B (Age 2) walked passed staff A and staff B towards the gate unseen by staff and was able to exit the playground. The child was later found on the parking lot sidewalk of the facility by a parent. Staff were not aware that child A was not in their care until the child was brought back to them by the center director.
Plan of Correction: Retrain all staff on playground safety and supervision rules.

Based on interviews it was determined that children did not always remain in sight and sound supervision of staff. Evidence: On 08/23/2022 at 4:45 pm a parent in the parking lot of the facility observed child A (Age 2) walking alone on the sidewalk area in the parking lot. The Parent retrieved the child and brought them into the facility to the center director.
Plan of Correction: Review supervision and gate safety.
