Inspector Notes

An investigation was initiated from 11:30 am - 1:15 pm in conjunction with a monitoring inspection to investigate a self-reported incident involving supervision. Interviews were conducted, video footage, documentation, child records, and staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-340-A
Based on interviews and video footage it was determined that staff did not always ensure the care, protection, and guidance, of children while in their care. Evidence: On 10/19/2023 at approximately 9:44 am the preschool one classroom was preparing to go outside. Staff A leads the students out of the classroom to the playground while Staff B follows at the end of the line. Approximately one minute later child A (Age 3) exits the bathroom of the classroom into the empty preschool classroom. The child reportedly looks around the classroom appearing confused. The child then sits at the block area and plays with some toys. The child then goes to the window to look at the other children on the playground. At approximately 10:16 am Staff C walks by the classroom and observes child A alone in the classroom. Staff C retrieves child A and returns them to their class on the playground.
Plan of Correction: Retrain all staff on face-to-name procedures.

Approximately one minute later child A (Age 3) exits the bathroom of the classroom into the empty preschool classroom. At approximately 10:16 am Staff C walks by the classroom and observes child A alone in the classroom. Staff C retrieves child A and returns them to their class on the playground. It was reported that staff A and staff B did not conduct the required name to face attendance accountability before leaving the classroom and once they arrived outside.
