Inspector Notes

A monitoring inspection was initiated on 09/11/2023 and concluded on 09/11/2023. There were 18 children present, ranging in ages from 5 years to 9 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records, 1 medication authorization and 4 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-240-A
Based on record review and interview, the center failed to ensure that staff completed the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. The record of Staff 2 (DOH: 04/13/2023) did not contain documentation of the 10-hour VDOE sponsored orientation course.
2. Staff 5 confirmed that Staff 2 did not complete the course.
Plan of Correction: I will make sure that the new staff complete the orientation within the correct timeframe.
Standard 8VAC20-780-240-E
Based on record review and interview, the center failed to ensure that all staff complete orientation training in first aid and cardiopulmonary resuscitation within 30 days of the first day of employment.

Evidence:
1. The record of Staff 4 (DOH 06/21/2023) does not have documentation of orientation training in first aid (FA) and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.
2. Staff 5 confirmed that Staff 4 did not have the FA and CPR training within 30 days of the first day of employment.
Plan of Correction: I will make sure that we cover this and initial it on the orientation form for documentation.
