Inspector Notes
An unannounced monitoring inspection was conducted on 7/21/2023 from 10:45am to 1pm. There were 128 (Infants to School-Age) directly supervised by 21 staff. The physical plant, 5 staff records, 5 children?s records, 4 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, preparing for and eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Violations
4Evidence: observed in the Outback classroom, staples protruding from the wall, with one end of the staple in the wall and the other end sticking straight out. Observed in the Outback classroom, a push pin low on the wall accessible to children under 3 years of age.
Evidence: observed the the following items:
1) observed several bottles of shaving cream in the Outback room, accessible to children
2) observed rubbing alcohol and shaving cream in an unlocked cabinet accessible to children in the School-age room.
Evidence: observed antacids/pan relief tablets accessible to children in an unlocked cabinet in the school-age classroom.
Evidence: reviewed the emergency evacuation drill and found that they did not practice a drill in the month of May 2924
