Inspector Notes
An unannounced monitoring inspection was conducted today in response to a self reported incident that was received by the Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegations of lack of supervision. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of lack of supervision, and the incident is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days. Violations
5Evidence: reviewed 2 staff records and found that both Staff 1 and 2 did not have documentation of orientation training.
Evidence: interviewed 2 staff members and found that Staff 1 and Staff 2 left Child 1 unsupervised in the classroom, while taking the other children to the playground. Staff were not aware that the child was missing until approximately 20-25 minutes later.
Evidence: Staff left Child 1 unsupervised for approximately 10 minutes, when they took the other children in the class to the playground.
Evidence: interviewed 3 staff members and found that Staff 1 and Staff 2 did not properly complete the facility's policy of name-to-face attendance, which resulted in staff leaving Child 1 unsupervised for approximately 10 minutes.
Evidence: reviewed 2 staff records and found that Staff 1 did not have documentation of 2 reference checks.
