Inspector Notes

An unannounced monitoring inspection was conducted today in response to a self reported incident that was received by the Office of Child Care Health and Safety. Interviews were conducted with staff relating to allegations of lack of supervision. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegations of lack of supervision, and the incident is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days. Based on review, the facility did not ensure that they had documentation of staff completing orientation training.
Evidence: reviewed 2 staff records and found that both Staff 1 and 2 did not have documentation of orientation training.

Based on staff interviews, staff did not ensure that when supervising children, they are ensuring their care, protection, and guidance.
Evidence: interviewed 2 staff members and found that Staff 1 and Staff 2 left Child 1 unsupervised in the classroom, while taking the other children to the playground. Staff were not aware that the child was missing until approximately 20-25 minutes later.

Based on staff interviews, the facility did not ensure that children under 10 years of age shall always be in actual sight and sound supervision of staff.
Evidence: Staff left Child 1 unsupervised for approximately 10 minutes, when they took the other children in the class to the playground.

Based on staff interviews, the staff did not follow the centers procedures of ensuring that children are receiving consistent care from staff.
Evidence: interviewed 3 staff members and found that Staff 1 and Staff 2 did not properly complete the facility's policy of name-to-face attendance, which resulted in staff leaving Child 1 unsupervised for approximately 10 minutes.
