During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated and completed on 5/02/2025 in response to a self-report received by the licensing office on 4/28/2025 relating to supervision. The inspector reviewed one child?s record and two staff records on-site. Four staff members were interviewed.
This inspection included document review (i.e. policies and procedures, staff training, and background checks), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/21/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-A
(Repeat Violation) When staff are supervising children, they shall always ensure their care, protection, and guidance. Staff #1 and Staff #2 did not ensure the care, protection, and guidance of Child #1 on 4/28/2025. Child #1, age 22 months, was able to slip out the classroom door to the playground unnoticed at 4:00 p.m. Video footage showed Child #1 alone and unsupervised on the playground for 12 minutes. Staff #3 took the child back to her classroom at 4:12 p.m.

Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff. Child #1, age 22 months, was out of sight and sound supervision of staff from 4:00 p.m. until 4:12 p.m. on 4/28/2025. Child #1 slipped out the classroom door to the playground unnoticed at 4:00 p.m. where she remained until 4:12 p.m. Staff #3 from another classroom saw her and took her back to her classroom at 4:12 p.m.. When Child #1 left the classroom, Staff #1 reported she had been changing a diaper and talking with a parent that arrived to pick up another child. Staff #2 reported she had been reading a story to a group of children and was not facing the door. Neither staff member saw the child leave or heard the door shut.
Plan of Correction: Door chimes installed. Active supervision training (all staff). Additional classroom observations/unannounced supervision checks. Child Supervision Record training (all staff).
Standard 8VAC20-780-40-E
(Repeat Violation) The operational responsibilities of the licensee shall include ensuring that the center's services are maintained in compliance with the center's own policies and procedures that are required by these standards. Staff #1 and Staff #2 did not follow KinderCare?s policies regarding Name-to-Face Checks. KinderCare policy states that ?Aside from transition times, you must complete and document name-to-face checks on the Child Supervision Record eight times a day.? No Name-to-Face Checks were completed in the Pond room at 4:00 p.m. or 5:00 p.m. as required on 4/28/2025. Child #1, age 22 months, left the classroom through the door that opens to the playground at 4:00 p.m. Video footage showed Child #1 alone and unsupervised on the playground until Staff #3 from another classroom saw her at 4:12 p.m. Staff #3 then took Child #1 back to her classroom.
