Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 8/21/2025 and completed on 9/03/2025 in response to a self-report received by the licensing office on 7/15/2025 relating to forbidden actions. The inspector reviewed 10 children?s records and five staff records on-site. This inspection included: document review (incident investigation and summary, background checks, training documentation, and complaint documentation), tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
