Inspector Notes
An unannounced monitoring inspection was conducted today from approximately 10:50am-12:00pm. There were 6 children in the direct care and supervision of 2 caregivers. A sample of 3 children?s records, and 2 caregiver's records were reviewed. The children were observed playing, engaged in an art project, eating and dancing. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. 6Evidence:
Caregiver A stated that she and Caregiver B did not complete 16 hours of annual training in 2022.
Evidence:
The documentation of attendance taken in 2022 was not kept.
Evidence:
The first aid kit was missing 1 of 2 required triangle bandages.
Evidence:
Caregiver A stated she did not have extra batteries for the radio.
Evidence:
The facility did not maintain documentation of emergency evacuation and shelter-in-place drills for 2022.
Evidence:
3 bottles located in the refrigerator and two bottles located on the counter top were not labeled with the child's full name and the date.
