Inspector Notes

An unannounced monitoring inspection was conducted on 10/25/23 from 11:00 am to 11:40 am. There were 4 children (8 points) in care of one caregiver. The children were observed playing with toys, singing songs and counting numbers. There were no children in care with medications or allergies on site. 2 children's records, 1 caregiver's, 2 household member's records, emergency drill records and attendance were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on staff interview, the family day home did not obtain results of a fingerprint based criminal record check (CRC) for an adult household member.
Evidence: The provider stated that she did not complete the CRC for Household Member # 1 when they turned 18 years old.
Plan of Correction: Household Member # 1 was unavailable when they turned 18. The search will be completed and the licensing inspector will be notified.

Based on staff interview, the family day home did not obtain a Sworn Disclosure Statement (SDS) for an adult household member.
Evidence: The provider stated that she did not have Household Member # 1 complete a SDS when they turned 18 years old.
Plan of Correction: Household Member # 1 was unavailable when they turned 18. The form will be completed.
Standard 8VAC20-800-170-A
Based on staff interview, the provider did not obtain from each adult household member a current Report of Tuberculosis (TB) Screening.
Evidence: The provider stated that she did not have a current TB screening for Household Member # 1.
Plan of Correction: The TB screening will be completed.
