Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 10/23/24 from 8:50 am to 9:40 am. The inspector reviewed compliance in the areas listed above.There were 5 children present and 1 caregiver; the point total was 13. The inspector reviewed 4 children?s records, 1 caregiver, and 1 household member's record on-site. This inspection included document review, tour of the family day home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Child # 1 has been in attendance for over two months. Child # 2 has been in attendance for over one month. Documentation of a physical exam has not been obtained for the children to this date.
Plan of Correction: Missing records will be obtained.
Standard 8VAC20-800-110-B
Children's immunization records shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
Child # 1's immunization records were not signed and dated by a physician.
Plan of Correction: Missing records will be obtained.
Standard 8VAC20-800-690-E
Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
After changing a diaper, the provider placed the soiled diaper on a packet of wipes sitting next to the diaper changing table.
Plan of Correction: The diaper pail will be moved closer to the diaper changing table. Diapers will be disposed off in the diaper pail.
