Inspector Notes

An unannounced monitoring visit was conducted for the Aspire afterschool program. 4 rooms were in attendance with direct care staff in ratio for supervision. Observed children transitioning from snack (popcorn chicken, melon, roll and milk) to tutoring and reading. No injury forms have been completed since the previous visit. Two emergency medications were on-site with 3 staff having current Medication Administration Training (MAT) certification. All staff have First Aid and CPR training. Bathrooms were equipped with required supplies. No new staff since the previous visit. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on medication review, one long term medication does not have written authorization from the child's physician.
Evidence:
Child #1 has the parent authorization but no written authorization from the physician for a prescription medication kept on-site longer than 10 days.
Plan of Correction: Another copy of hte Physician form was given to the parent on 3/22/2023.

Based on record review, the center failed to implement a monthly practice evacuation drill.
Evidence:
No documentation was recorded that drills were conducted for November 2022, December 2022 and January 2023.
Plan of Correction: Program staff will be reminded to enter information for all emergency drills.
