Inspector Notes

An unannounced renewal inspection was conducted on 04/26/2022 from approximately 8:00 am to 8:30 am. There were 9 children supervised by 3 direct care staff.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records were reviewed.
Children were observed during arrival, participating in group play, reading, and playing on the gym.

Based on record review, required staff records were not kept for each staff person.
Evidence:
1) The following documentation is required to be kept at the center for each staff person and available for review: employment date, emergency contact, cpr/fa certification dates, MAT/DHO certification dates, dates for the Health and Safety Training and updates. On the date of inspection, any of the required documentation was available for review for Staff #1.
Plan of Correction: Receive copies of records from staff member and supervisor.
