Inspector Notes
An unannounced subsidy health and safety (SHSI) inspection was initiated on 03/23/2022 and concluded on 03/29/2022. There were 10 children present with three staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of three staff records and interviews, the vendor did not ensure one staff member had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment.
Evidence: 1) The record for Staff #3, hired on 12/08/21, did not contain documentatin that the staff member has current certification in CPR. 2) During interviews, a member of management confirmed Staff #3 does not have current CPR ceritification.
