Inspector Notes
An unannounced monitoring inspection was initiated on 02/04/2025 and completed on 03/20/2025 with a local agency, in response to a self-report received by the licensing office on 02/04/2025 relating to supervision and forbidden actions. The inspector reviewed one child?s record on-site on 02/06/2025. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
