Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 06/25/25 and completed on 06/30/25, as a part of the licensure period. The onsite inspection began at 10:35am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 26 children present and five staff. The inspector reviewed five children?s records and five staff records on-site. Additional documentation was reviewed electronically on 06/30/25. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

The repeat TB screening in the record of Staff #2 was completed 14 days late.
Plan of Correction: Corrected. In the future, staff will complete the repeat TBs prior to expiration.
Standard 8VAC20-780-245-A
Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

The record of Staff #3 did not contain documentation that the staff completed annually a minimum of 16 hours of training from April 2024-April 2025.
Plan of Correction: Staff #3 is working on completing annual training. Staff will complete 16 hours of annual training moving forward.
