(Repeat Violation) A tuberculosis (TB) screening is required every two years from the last screening.

The TB in the record of Staff #3 was overdue by more than two months.

Standard 8VAC20-780-245-A
(Repeat Violation) Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care.

The record of Staff #3 did not contain documentation that the staff completed annually a minimum of 16 hours of training from August 2024-August 2025.

Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes the required documentation.

The emergency evacuation drills conducted in July, August, September, and October 2025, did not contain the method used for notification of the drill; the number of staff participating; any special conditions simulated; the time it took to complete the drill; or weather conditions.
Plan of Correction: Per the Center: "We have 2 pages for this - Inspector maybe didn't see as we have them in separate parts of binder - Director was unable to be there & didn't point it out. Will send pictures of both pages that are up to date for all requirements."

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the required information.

Child records shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. The record of Child #1, who has been enrolled for more than 10 months, did not have the address for the second emergency contact listed.
