Inspector Notes
A renewal inspection was initiated on June 29, 2022 and concluded on June 29, 2022. There were 52 children present, ranging in ages from Infant to Preschool, with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
7Evidence:
1. The record for staff #2 (hire date 05/02/2022) did not contain documentation of central registry findings.
2. The record for staff #3 (hire date 03/21/2022) did not contain documentation of central registry findings.
Evidence:
The records for child #2 (start date 1/03/2022), did not have a physical examination.
The record for staff #2 and staff #3 did not have documentation of orientation training which is appropriate to the age of the children in care.
Evidence:
1. Child A was on the changing table when Staff A turned and walked approximately 3 feet away from Child A and the changing table to talk with another teacher.
2. Child A was left unsupervised while laying on the changing table which was approximately four feet above the floor.

The director stated that the substituted lunch foods were not posted.

1. There was four bottles of infant formula that was not dated.
2. Staff confirmed that the bottles were not labeled.
Evidence:
1. The record for child #1 (start date 6/27/22), did not contain one of the two emergency contacts.
2. The record for child #2 (01/03/22), did not contain parental agreements between the parent and the center which shall be in child's record by the first day of the child's attendance.
3. The record for child #3 (05/09/22), did not contain proof of birth.
