During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on June 3, 2025 and completed on June 3, 2025. The on-site inspection began at 11:15 AM and ended at 2:50 PM. The inspector reviewed compliance in the areas listed above. There were 102 children present and 18 staff. The inspector reviewed ten children?s records and ten staff records onsite. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, on June 12, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

The record for child #3 had documentation of immunization 49 days after the required timeframe.
Plan of Correction: Created a better system of enrollment to make sure no child starts w/o proper documentation.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

The record for child #3 had documentation of a physical examination 19 days after the required timeframe.
Plan of Correction: Created a better system of enrollment to make sure no child start w/o physical and all work present.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #10 updated screening was completed 114 days after the required update was due.
Plan of Correction: Went through files and system in place to make sure no TB or important information is missed.

Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Staff #1 had no documentation of completed training and had been employed for 11 days.

Sunscreen shall be inaccessible to children under five years of age.

In the two's room, containers of sunscreen were observed in two children's cubbies.
Plan of Correction: Trained the teachers to lock in bins daily and remove items such as sunscreen and bug spray.

Insect repellent shall be inaccessible to children.

In the two's room, one container of insect repellent was observed in a child's cubby.
Plan of Correction: Completed a training to ensure that the teacher checks bins daily to make sure their are no items such as sunscreen or insect sprays in bin.

Prepared infant formula shall be dated and labeled with the child?s name.

Nine infant bottles with formula were observed not dated in the refrigerator.
Plan of Correction: Provided different color tape to indicate B.milk and formula. Which also allows the teacher to add date if parent do not.
