During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on July 21, 2025 and completed on July 31, 2025 in response to a self-report received by the licensing office on July 11, 2025 relating to supervision of children. The inspector reviewed four staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to background checks and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 12, 2025. A POC submitted after this date will not appear on the public website.
Violations
4The record for staff #3 did not have documentation of a sworn statement. The sworn statement is overdue by 3 years, 6 months.
1. On July 11, 2025, video footage captured room #1 with eight children (ages: 16 months to 24 months), alone and unsupervised for 44 seconds.
2. Staff #2 exited room #1 through the back door and 20 seconds later staff #1 exited room#1 through the front door. Staff #1 and staff #2 did not realize the other staff member had exited room #1.
3. Staff #2 and staff #4 stated that the children in room #1 were left alone for less than a minute.
1. On July 11, 2025, during the designated resting period, video footage captured eight children (ages: 16 months to 24 months), alone and unsupervised in room #1 for 44 seconds.
2. Staff 2 stated when she return to room #1, staff #2 was not in room #1.
1. In room #1, during the designated rest period, eight children (ages: 16 months to 24 months) were resting on cots alone without the supervision of a staff member.
2. The center's policy states in each staff member's job descriptions for general classroom duties: provide consistent and constant supervision of all children. Children should never be left alone.
