During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 9, 2025 and completed on October 9, 2025. The on-site inspection began at 9:25 AM and ended at 12:45 PM. The inspector reviewed compliance in the areas listed above. There were 85 children present and 14 staff. The inspector reviewed eight children?s records and eight staff records. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 24, 2025. A POC submitted after this date will not appear on the public website.
Violations
41. In room #7, the bathroom had chipped and peeling paint within reach of children.
2. In room #9, one hole in the wall within reach of children and a mop bucket being stored with approximately 4 inches of liquid used to clean floors the day before.
A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
1. In room #1, the diapering pad was torn.
2. In room #7, interviews revealed that children younger than three years old have their diapers changed in the bathroom while standing up. The children are not placed on a changing table or countertop designated for changing.
Medication for Child A was onsite, but the medication authorization had expired over nine days ago. The authorization had not been renewed and the medication was not returned or disposed of as required.
1.The center's first aid kit was missing one triangular bandage.
2.The first aid kit on Bus #1 was missing a thermometer.
3.The first aid kit on Bus #2 was missing scissors, tweezers, and a thermometer.
