Inspector Notes

An unannounced monitoring inspection was initiated on October 9, 2025 and completed on November 19, 2025 in response to a complaint received by the licensing office on October 2, 2025 relating to Special Care Provisions and Emergencies. The inspector reviewed six children?s medication records and one staff record on-site. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Whenever the center agrees to administer prescription medication, a staff member must have completed a medication administration training program.

1. Staff #1 administered medication to Child D from 8/26/25 to 9/02/25 and child #F on 11/13/25.
2. Staff #2 Medication administration training had been expired for over 1 year and 5 months.

The medication authorization for a child shall be available to staff during the entire time it is effective.

The record for Child A did not contain a medication authorization for prescription medication stored for the child on-site.
