When staff are supervising children, they shall always ensure their care, protection and guidance.
1. Staff #1 sprayed disinfectant on the toilet seats and did not dilute the disinfectant as directed on the product label. Staff #1 did not remove excess liquid once she completed disinfecting.
2. Child #1, child #2, child #3 used the toilet seats after staff #1 sprayed the disinfectant. Child #1, child #2, and child 3# reported irritation to their buttock.
