Inspector Notes

The following inspection took place between the hours of 10 am through 3 pm. Today, I met with the Staff of the Everbrook Academy located near Arcola, Virginia. At the start of this announced initial inspection there were a total of 29 children observed with 7 classroom staff; good ratios in all 5 rooms. A total of 5 children?s files and 4 board officers/admin files were reviewed. The children were engaged in the following: story time, imaginative play in the plaza, outside time on the playground, center activities within their classroom, lunch, infant sleeping, and other organized activities. Standard 22VAC40-185-500-B
Based on interview, a nonabsorbent surface (a changing table or countertop designated for changing) for diapering or changing was not used for children younger than three years.

Evidence -
1. The Center Director stated (during an interview conducted on the date of inspection - 10/05/2021) that children within the 2's room are changed using their standing up procedures.
2. Neither a changing table, nor a countertop designated as a changing surface was observed within the 2's room.
Plan of Correction: A changing table shall be purchased for this room. In the meantime, teachers shall escort the children as a group to be changed in a nearby classroom that has a changing table.
Standard 22VAC40-185-570-E
Based on observation, infant formula being refrigerated within the Toddler classroom was not labeled correctly.

Evidence -
1. On the date of inspection (10/05/2021), 2 of 2 bottles observed (while having the name of each child) were missing the current date.
2. This observation was confirmed on the date of inspection by the Center Director.
Plan of Correction: Both staff and parents shall be notified to remind them of the correct form of labeling.
