Inspector Notes

A non-mandated monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

This inspection was initiated and concluded on 02/23/2022. The Director was contacted and a virtual inspection was conducted. There were 8 children present (at 8:20 am), ranging from infant to preschool age, with 9 staff supervising, in 5 rooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 10 staff records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with an applicable standard, and an applicable code. These violations were documented on the Violation Notice issued to the program.

Standard 22.1-289-036-B-4
Based on review, 3 of 10 staff files indicated that the Child Day Center (CDC) did not obtain a copy of the results of a Criminal history Record information Check (CRC), a Sex Offender Registry check (SOR), and a search of the child abuse and neglect registry or equivalent registry (CPS) from any state in which the individual has resided in the preceding five years.

Evidence -
1. On the date of inspection (02/23/2022), the following staff did not have the following documentation available for review:
Staff #2 (State of Florida - SOR) start date - 10/15/2020;
Staff #7 (State of California - CRC, SOR, CPS) start date - 09/07/2021
Staff #8 (State of Ohio - SOR) start date - 01/24/2022
2. This finding was confirmed on the date of inspection by the Center Director.
Plan of Correction: Requests for the CPS check from California has been sent. All other SOR and CRC will be sent within 7 days. Once these items have been obtained they will be reviewed by Licensing Inspector.
Standard 8VAC20-780-130-E
Based on review, 1 of 5 children files indicated that the center did not obtain documentation of additional immunizations once every six months for a child under the age of two years.

Evidence -
1. On the date of inspection (02/23/2022), documentation of an immunization page for Child #2 was dated - 07/22/2021.
2. Based on the information on file for Child #2, on the date of inspection, Child #2 was under the age of two years.
Plan of Correction: The Parent of Child #2 will be contacted and an updated immunization page obtained.
