Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 10/27/2022, between the hours of 9:30 am to 1 pm. There were 30 children present, ranging from infant to preschool-age, with 7 staff supervising, across 4 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 10 staff records were reviewed.

Information gathered during the inspection determined that there were (5) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Standard 8VAC20-780-240-A
Based on review, 1 of 10 staff files indicated that the Virginia Department of Education-sponsored orientation course was not completed within 90 calendar days of their start date.

Evidence -
1. On the date of inspection (10/27/2022), a certification of completion of the Virginia Pre-service training (10 hours) was not available for review for Staff #6.
2. The start date for Staff #6 was listed as 03/14/2022.
Plan of Correction: All missing certifications will be obtained.
Standard 8VAC20-780-280-B
Based on observation, cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence -
1. On the date of inspection (10/27/2022), cleaning supplies (a spray bottle containing sanitizing fluid) was found in an unlocked cabinet within the preschool room.
2. This bottle was not in use at the time of observation.
Plan of Correction: Cleaning supplies will be locked up.
Standard 8VAC20-780-500-A
Based on observation, hand washing procedures were not followed.

Evidence -
1. On the date of inspection (10/27/2022), within the Toddler/2's room, after a staff person completed a diaper change of a child, neither the staff nor the child had their hands washed.
2. This observation was confirmed by the Center Director.
Plan of Correction: Hand washing procedures will be followed.
Standard 8VAC20-780-530-A
Based on review and observation, at least one staff in each classroom or area where children were present did not have a certification in CPR and First Aid.

Evidence -
1. On the date of inspection (10/27/2022), certification in CPR/First Adi was not available for review for Staff 10. The start date for Staff #10 was listed as 06/20/2022.
2. On the date of inspection, Staff #10 was observed providing care in the Infant Room for one child.
Plan of Correction: Certification in CPR/First Aid will be obtained and placed on file.
Standard 8VAC20-780-70
Based on review, 1 of 10 staff files indicated that not all of the required documentation was on file for each employee.

Evidence -
1. On the date of inspection (10/27/2022), 0 of 2 references were on file for Staff #8.
2. The start date for Staff #8 was listed as 10/03/2022.
