Inspector Notes
This monitoring inspection was initiated by licensing staff and concluded on 04/25/2023, between the hours of 9:30 am to 12:30 pm. There were 46 children present, ranging from infant to preschool-age, with 10 staff supervising; across 5 separate classrooms. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records, and 4 staff records were reviewed.
Information gathered during the inspection determined that there were (4) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

4Evidence -
1. On the date of inspection (04/25/2023), a certificate of completion for the Virginia Preservice training (10 hour course) was not available for review for Staff #11.
2. The start date for Staff #11 was listed as 11/14/2022.
Evidence -
1. On the date of inspection (04/25/2023), documentation of orientation training for Staff #11 was not available for review.

Evidence -
1. On the date of inspection (04/25/2023), a wall in the Pre-K Room showed signs of peeling paint. The surface of the drywall appeared torn in one area in the shape of a large triangle, and another smaller circular area higher up the wall.
2. The damaged area was being blocked by a large white cube at the time of inspection.
Evidence -
1. On the date of inspection (04/25/2023), two spray bottles of cleaning fluid was observed out on the counter, near the diapering surface within the Toddler Room.
2. These two bottles were not in use and children were not in the room at the time of observation.
